UCLA Waiver of Liability, Assumption of Risk, Indemnity Agreement, and Photography/Video Notice

ALL participants must read the following and sign the "Acknowledgment of Understanding" below, including parents on behalf of all minor children.**

WAIVER OF LIABILITY & ASSUMPTION OF RISK

WAIVER OF LIABILITY:

In consideration of being permitted to participate in any way in Water Skiing, Boating, Fishing, Hiking, Mountain Biking, Archery, Swimming, Ropes Course, Softball, Football, Volleyball, Tennis, Basketball, Cardio Kickboxing, Yoga, Pilates, and any other activity associated with or undertaken at The UCLA Lake Arrowhead Lodge, hereinafter called “The Activity,” I, for myself, my heirs, personal representative or assigns, do hereby release, waive, discharge, and covenant not to sue the Regents of the University of California, its officers, employees, and agents from liability from any and all claims including the negligence of The Regents of the University of California, its officers, employees and agents, resulting in personal injury, accidents, or illnesses (including death) and property loss arising from, but not limited to, participation in The Activity.

ASSUMPTION OF RISKS:

Participation in The Activity carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another, but the risks range from:

minor injuries such as scratches, bruises, and sprains;
major injuries such as eye injury or loss of sight, joint or back injuries, heart attacks, and concussions;
catastrophic injuries including paralysis and death.

I agree that I am voluntarily participating in the Activity and understand the risks involved with the Activity. I acknowledge that I am physically fit with no physical limitations or medical condition that would preclude my safe participation in the Activity. While I understand that The UCLA Lake Arrowhead Lodge will be providing general instruction on the Activity, I understand that I have a primary role in ensuring my safe participation in the Activity and can cease the Activity at any time.

INDEMNIFICATION AND HOLD HARMLESS:

I also agree to INDEMNIFY AND HOLD The Regents of the University of California HARMLESS from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees brought as a result of my involvement in The Activity and to reimburse them for any such expenses incurred.

SEVERABILITY:

The undersigned further expressly agrees that the foregoing Waiver and Assumption of Risks agreement is intended to be as broad and inclusive as is permitted by the law of the State of California and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

CODE OF CONDUCT

ALCOHOL POLICY:

At UCLA Lake Arrowhead Lodge, guests twenty-one (21) years or older may enjoy alcoholic beverages in a responsible manner, only as follows:

Alcohol must be purchased from, and consumed in, the Lodge’s tavern or restaurant.
Personal alcohol (and alcohol containers) must remain within your guest room, with the exception of wine bottles, which you may bring to the Lodge’s restaurant only.
If you choose to bring wine to the restaurant, you will be charged a $15 corkage fee, per bottle.

GUEST BEHAVIOR:

We expect all guests to conduct themselves with respect and courtesy towards others. Physical violence, verbal aggression, threats, and disruptive conduct are strictly prohibited. Harassment or discrimination based on race, gender, age, disability, or any other characteristic will not be tolerated. Overconsumption of alcohol leading to disruptive behavior and the unauthorized use of personal alcohol at programmed events or public areas outside designated spots is not allowed. Guests are responsible for their actions. Violations may result in removal from UCLA Lake Arrowhead Lodge without refund and/or exclusion from future programs at UCLA Lake Arrowhead Lodge.

GUEST AGREEMENT:

By signing below, I acknowledge that I have read, understood, and agree to abide by these UCLA Lake Arrowhead Lodge policies. I understand that failure to comply may result in disciplinary actions, including removal from the premises and exclusion from future programs. Refusal to sign will prohibit participation in the 2025 The UCLA Lake Arrowhead Lodge program, and you may be asked to leave the property.

RECORDING NOTICE

PHOTOGRAPHY/VIDEO NOTICE:

Photographs and/or video recordings will be taken during The UCLA Lake Arrowhead Lodge. I consent to the use of these photographs and/or video recordings taken of my name, image, or likeliness while attending The UCLA Lake Arrowhead Lodge for the purpose of advertisements and communications for The UCLA Lake Arrowhead Lodge.
